半导体激光美白牙齿 半导体激光在牙齿美白术中的应用

  [摘要]目的:了解半导体激光在牙齿美白术中的应用。方法:随机选择78例患者,女性患者比男性多,约4:1,平均年龄33岁。35%H2O2作为美白剂,美白过程中以半导体激光进行牙齿美白催化。结果:经过4个循环近1h的牙齿激光美白,牙齿在色阶上有显著性差异,美白过程中,患者有不同程度酸痛感,24h后不适感消失。结论:半导体激光美白牙齿效果显著,对周围组织刺激性小,临床易于推广应用。

  The clinical study by lasers-assisted in tooth bleaching

  Abstract:ObjectiveTo understand the effect of the clinical trail study by diode Lasers-assisted in tooth bleaching. Methods eighty patients at random (Average age was 33 years) Who had done in office diode laser-assisted tooth bleaching with 35%H2O2,data were collected.Results After four rounds diode-assisted tooth bleaching, there are significant changes with the teeth shade. During the tooth bleaching procedure, patients felt slightly tooth sensitivity.After 24 hour,there is no patient has any tooth sensitive.Conclusionthe effect of tooth Laser-assisted bleaching is satisfied,The patient has less tooth sensitive for treatment,it could be used in clinical study.

  自从1960年T.H Maiman发现红宝石激光以来,激光在口腔医疗领域内得到广泛应用。有人将半导体激光应用在口腔医学领域内,其开始被用于软硬组织切割及美白牙齿,备受口腔医学界的重视,然而对半导体激光美白牙齿的作用机制及实际效果缺乏系统而完整的研究。本文通过78例患者采用半导体激光美白牙齿,探讨其效果,给口腔科医师提供牙齿美白的临床参考。
  
  1材料和方法
  
  1.1本次临床研究使用的半导体激光器是选用美国Biolase公司产的Twilite系统,该机其激光之介质为半导体二极管,波长(815±15)nm,能量0~10W,波形可连续、重复或单一,激光发射时间20ms~9.9sec;美白剂采用该公司生产的35%H2O2。
  1.2 临床实验对象是门诊就诊要求行美白的患者,采用随机抽样方式抽样,样本总数为80名,其中男性17人(22%),女性61人(78%),美白前平均色阶为12.6±3,经过激光美白后平均色阶为2.33±1.42。由本实验中可知道这段时间内做牙齿美白的患者女性比男性多,男、女比例为1:4,年龄分布于21~51岁,男性平均为37.3岁,女性为32.1岁,其中以20~29岁占38%,30~39岁占35%,40~49岁占20%。负责收集病例的医护人员在临床上接受统一训练,牙齿美白前后比色只接受两位受训医师比色,在研究开始前两位医师在受训后也先进行相同10位一般患者牙齿比色测试,结果两位医师的相关系数为0.9,比色测量表是以Vita-shade色泽表进行比色[2](见表1)。美白室的环境为白光,记录美白所用的摄影机为35mm专业摄影机,并且固定摄影条件而负责收集资料及临床实验医护人员采用严格、统一的通用标准,美白比色及操作统一固定,尽可能减小操作过程中的误差。
  1.3美白方法及机制:采用统一标准的半导体激光牙齿美白步骤和方法,结束后以门诊问卷记录患者的基本资料,美白前后色阶及在美白过程中反应(无痛/微酸/很酸/痛/很痛),并请患者于3日后回诊检查并记录美白当天及隔天是否有不适反应。其机制采用高浓度美白剂通过氧化作用,并用半导体激光予以催化,其最适应的酸碱度为pH9.5~10.8,有最强的美白效果。
  1.4临床牙齿美白后以问卷记录患者的基本材料,美白前后之色阶及美白过程中反应(无痛/微酸/很酸/痛/很痛)并请患者于3日后复诊检查并记录美白后当天及隔天是否有酸痛问题。
  1.5有关资料收集后以SASFor Windows 8.11版进行资料分析,包括计算人口学分布特征,色阶差异性,病患牙齿酸痛反应及其满意度等,再进行t-test ANOVA回归等推理性统计处理。

  由本实验中可知道这段时间内做牙齿美白的患者女性比男性多,无论是男性或女性激光美白牙齿前后色阶有显著性差异(P
