基底冠什么意思 [三种烤瓷基底冠适合性的比较研究]

  [摘要]目的:研究比较Captek、贵金属合金和镍铬合金3种不同烤瓷基底冠适合性,为临床应用提供实验依据。方法:分别制备Captek、贵金属合金、镍铬合金基底冠,用磷酸锌粘固剂将3组试件粘固于代型上,用线切割机将冠及代型沿纵轴抛开,采用扫描电镜测量6个观察点处基底冠组织面与代型之间的间隙大小,其值进行统计学分析,比较3组基底冠不同部位的适合性。结果:Captek的间隙最大值为(98.02±4.10)μm,贵金属合金基底冠的间隙最大值为(102.42±6.51)μm,镍铬合金基底冠的间隙最大值为(135.67±5.30)μm。Captek和贵金属合金基底冠适合性在统计学上无明显性差异 (P>0.05),Captek和贵金属合金基底冠适合性均与镍铬合金基底冠有显著的统计学差异(P<0.05)。结论:Captek和贵金属合金基底冠具有良好的适合性,与上述二者相比,镍铬合金基底冠边缘适合性较差。

  Comparative studies on adaptation of captek, noble alloy and Ni-Cr alloy

  Abstract: Objective To compare the marginal adaptation of three kinds of porcelain fused to metal base crown materials:Captek, noble alloy and Ni-Cr alloy. And it can provide an experimental basis for the clinic.MethodsCaptek, noble alloy and Ni-Cr alloy were selected to fabricate PFM base crowns. All base crowns were cemented to their respective dies by zinc phosphate cement. After that, these base crowns/dies assembly were cut along longitudinal axis by line cutting machine. The gap between base crown tissue surface and die was measured by 6-point measuring method with SEM.The marginal adaptation of different parts of three kind of base crowns were compared statistically. Results The values of adaption of three metales base crowns respectively are (98.02±4.10)μm,(102.42±6.51)μm,(135.67±5.30)μm. Statistical analysis of variance of marginal adaptation values showed that no significant difference was between Captek group and noble alloy group (P>0.05).However,there were significant difference between Ni-Cr alloy and the other groups(P   1.2.4 冠的粘固:在代型的轴壁上开辟一宽1mm的排溢沟,用磷酸锌粘固剂将全冠按编号分别粘固于相应代型上。粉液比为3:1。将调拌好的磷酸锌粘固剂用毛笔在冠组织面轻轻涂一层,指压就位后置于压力实验机上,载荷5Kg维持10min,去除冠边缘多余粘固剂。
  1.2.5适合性的测定:用线切割机将冠及代型沿纵轴抛开,切面经300、400、600目耐水砂纸磨光后以备测量。用JSM-6460型扫描电镜放大500倍测量冠的A1、B1、C1、A2、B2、C2 6个点基底冠组织面与代型之间间隙的大小,精确到0.1μm。(如图2)
  1.2.6 统计分析:将所测数值用SPSS10.0软件进行单因素方差分析,计算组间差异。
  
  
  2结果(见表1~2)
  
  Captek组与贵金属组的六点观测值之间均无统计学差异(P>0.05);Captek组与Ni-Cr合金组在6点观测值中有4点具有统计学差异(P<0.05);Ni-Cr合金组与贵金属组在6点观测值中有4点具有统计学差异(P<0.05)。
  
  3讨论
  
  修复体的适合性是指修复体组织面与牙预备体之间间隙的大小或二者间粘固剂的厚度[5]。它反映了修复体的精确程度和就位情况,是衡量修复体质量的重要指标。修复体的适合性不良时,冠边缘不密合,使得粘固剂暴露于唾液中,易产生边缘微漏,导致食物残渣的堆积和牙菌斑沉积,继而发生继发龋、龈炎,严重影响牙体、牙周组织的健康,并影响修复体的美观[6]。Marco[7]等认为修复体边缘是影响修复体寿命的最薄弱环节,也是修复体成败的关键。
  修复体的边缘适合性,临床上以肉眼不能看到、探针不易探测到为判断标准,美国牙科学会(ADA)标准规定临床上可接受的边缘差异为25~40μm [8-9],但实际中修复体很难达到这一要求。综合文献报道,现在一般认为临床上可接受的边缘差异为100~120μm [10-12]。由实验结果可知Captek与传统贵金属冠的边缘适合性的大小在临床上是可以接受的,且二者无显著差别。Ni-Cr合金在A1、C1、A2、C24点与传统贵金属合金和Captek存在显著差异。三者在B1和B2点适合性很好,无差别。
  对铸造合金而言,影响修复体适合性的因素很多,几乎遍布修复体制作的每个步骤。较为重要的有:①包埋料与合金间的匹配;②合金的铸造性能。
  Captek技术与失蜡法铸造截然不同,用可塑性的P箔和G箔先后围贴石膏代型,在烤瓷炉内烧结,P层构筑的紧密空隙结构,经由G层熔融后渗透可形成复合基底冠。此过程中影响冠精度的主要是:①石膏代型的膨胀与Captek的膨胀是否一致;②技师围贴操作水平。
  Captek没有蜡型、包埋、铸造等环节,故可以避免由于蜡型变形,包埋料与合金不匹配以及铸造中因失误造成的修复体边缘适合性差,组织面有小瘤或表面缩孔等现象。Captek制作只有围贴和烧结两步骤,Captek厂商提供了一种石膏粘结剂和配套工具,P层与石膏代型的密贴,最大限度的保证了基底与代型的适合性,专用的耐火石膏的膨胀可以补偿P、G两层冷却中的收缩,而且G层,可以用专用工具高度抛光,一方面使边缘更密合,另一方面可以减少粘固后菌斑的附着,二者的共同的作用是龈炎发生率大大减少。因而captek具有良好的适合性,能满足临床要求。
