　 重点工程监督检查情况汇报

　 市纪委、监察局认真履行职能，紧紧围绕市重点工程建设项目开展全方位监督检查，努力把重点工程打造成“精品工程、效益工程、廉政工程”。8月28日－30日，我们组织了2006年重点工程（交通工程）第一次监督检查，现将本次监督检查情况汇总如下：

　 一、精心组织定方案

　 我们围绕如何确保市委、市政府的决策得到贯彻落实、政府重点工程优质、低价、廉洁进行了认真的研究，总结和借鉴兄弟单位对重点工程监管的先进做法，更新观念，拓宽思路，分析研究了当前建设领域存在的种种不规范现象，明确了今年重点工程监督思路。这次督查活动由纪委副书记、常委带队，实施抽查前制订了详细的工作方案，还专门召开了全体督查人员会议，明确了督查内容和督查重点，强调了相关纪律。为了保证检查活动的客观公正，特地邀请了具有专业资质的2名××监督员，4名××的特邀监督员，以及财政局的2名专职财会人员，分三组抽查了沿江公路C、D、E标段；农村公路中的麒兴路、中南北路和粮站北路；农桥中的镇张家桥、镇骑岸桥、镇祖平桥建设及质量情况。检查的内容包含：（1）资金的管理和使用情况；（2）项目的招投标情况；（3）合同的签订情况；（4）施工进度情况（5）工程质量、监理履责、工程验收等情况。

　 二、认真监督严把关

　 这次我们对重点工程督查，不通知业主单位，不在施工单位用餐，着力体现减少重点工程建设中的“长官意志和人为因素”、加强重点工程事中监督，克服了以往监督人员专业知识缺乏、跟着建设单位、施工人员转，随大流，走过场的现象，改变了传统的监管方式，较好地做到了对重点工程实施有效监督。所有参与督查人员都本着对党和人民负责的精神和高度的责任感投入到工作中，顶烈日、战高温、抢晴天。我们特邀的监督员对每个所检查的项目认真仔细，通过查阅台账、询问工程技术人员、现场察看等对工程质量能监到实处督到要害，指出的问题一针见血。财政资金专管员对重点工程资金的筹措、管理和使用进行了认真核对，点到的问题实事求是，引用的财务制度有根有据。我们对抽检的农村公路实施随机抽样钻孔检测，还对农桥工程所使用的水泥、钢材、黄砂、石子的查验、质保资料进行了检查，掌握了第一手真实资料，充分体现了对重点工程质量监督严格把关。

　 检查中发现所有工程都实行了招投标、签定了工程承包合同、安全合同、廉政合同，现场项目部各项规章制度均能在醒目位置公示。沿江公路三个标段的工程质量都处在有控制合格情况下，具体有待于检测试验数据予以证明。农桥的工程进度比较快，桥板、桥桩的预制件都已完成。3条农路的主体工程均已完工，工程观感质量良好，表面平整，砼施工无蜂窝麻面现象，道路宽度和钻芯厚度均符合设计要求。在资金的管理和使用方面，海晋、蓝天、鹏天三个公司财务凭证要素比较规范，资金无拉借现象。审批手续基本齐全，审批流程顺畅。沿江公路工程C、D、E标段，拨款手续完备，会计凭证中科目运用正确。

　 三、督查中发现存在的问题

　 （一）沿江公路部分标段进度较慢

　 沿江公路C标段工程进度太慢，路床全长6813米，目前只完成了5100米，占工程总量的75%；12%灰土层第一层才完成2800米，合同工期预定10月20日完成下封，按目前进度估计目标工期尚不能实现。

　 （二）工程质量及台帐资料存在的不足

　 1、从检查情况看，沿江公路C、D、E标段的工程质量处在有控制合格情况下，具体有待于检测试验数据予以证明。其中用土土质按照指挥部文件应该换土解决，但现场挖开抽样试验时，土质偏沙，未达到要求。设计规定土塑指数为12—20%，基本上都达不到这个要求。设计规定的12%灰土要做R7试验，要做180天劈裂试验，建设方是同意不进行试验的，最好由设计院出个变更手续。

　 C、D、E标段的检测仪器均未标定。

　 2、C标段：①12%灰土第一层石灰消解不太充分，（石灰质量不好）；②5%灰土的含灰量80%为负偏差（但在规定允许范围内）。

　 3、D标段：①资料中填方路基只标了第几层，没有标清路床表面以下几公分到几公分，不便以后的查阅。②道路高程复测表未写明设计标高，无法判断是否合格。

　 4、E标段：①资料中土方回填部分，5%含灰量标明了层位，但压实度资料未标层位，以后容易搞混；②道路标高复测记录不是原始记录（是打印的）。

　 5、常乐中南北路、粮站北路缩缝切割深度不够，未做路肩；在较大道口、村民居住集中地的道口、高差较大的道口未作引坡，存在安全隐患。

　 （三）监理履责存在漏洞

　 1、沿江公路C、D、E标段的监理都是东南监理公司，在沿江公路C标段，监理以独立的第三方体现得不完善。主要表现在业主单位的分管局长和工程技术人员担任该工程的总监和总监助理，一旦出现责任追究，将会职责难分。西组主要存在工地例会开得少，对C标共开三次，一般应有每月一次，在提供的资料中，还出现了监理人员粗心大意的现象，抽阅中发现对石灰剂量的检测，原始数据都有，惟独未对照查阅确定灰剂量，也无此项表达。又如对K17＋230左压实度试验结果为9.7，应当是97%。

　 3、在D、E标段的开工报告审批中，对施工组织设计审核监理工程师签了字，但缺少总监签字。

　 4、在农桥检查中监理方面，未能提供钢筋质保复检资料、监理查验资料张家桥和货隆骑岸桥的监理口头上说有砼试块，但没有登记记录；张家桥未进行现场查勘出平面布置图，使施工时遇到碰线问题；监理工作中还存在粗心的情况，如在桥桩砼配比通知单中有笔误，将“预制桩”写为“灌注桩”。

　 （四）财务凭证手续不完备

　 在对××、××、××公司会计凭证抽查中发现，存在部分审批手续不全的现象。

　 ××公司财务凭证：①06年1月16﹟、17﹟、25﹟凭证付工程队工程款，只有报告和审批表，无收据。②06年1月50﹟凭证付贷款利息127813元，无审批手续，只有银行利息单。③06年7月28﹟、31﹟、33﹟、54﹟、59﹟、60﹟、69﹟凭证审批缺少纪检签字。

　 ××公司财务凭证：①06年1月36﹟凭证付东洲画院标志款106807.2元，其中56263.2元手续完备，50544元只有局长一人批，没有其余人审核；②06年7月20﹟凭证预付兴达公司50万元，纪检未审批。

　 ××公司财务凭证：①06年1月31﹟凭证少财政所租地费发票27992元；②06年7月10﹟凭证预付市大康石化供销经理部补助约5000元，少项目组和纪检审核。

　 四、几点意见和建议

　 1、对沿江公路C、D、E标段要强势推进施工进度，以确保对施工合同和对社会承诺的兑现。

　 2、狠抓各工序的质量控制，尤其在结构层施工阶段管理须进行全过程旁站。

　 3、针对施工资料、监理资料凌乱、不齐等现象，建议及时收集、整理、归档。

　 4、沿江公路C标段建议使用质量好的石灰，石灰要充分消解。

　 5、各标段使用仪器要标定。

　 6、麒麟麒新路的胀缝切割加宽至2-2.5cm，麒麟麒新路、常乐粮站路和中南北路的胀缝处理有待改进，缩缝须尽快清缝后灌油膏或沥青，两侧路肩培土须尽快落实，确保安全；在较大道口，村民居住集中地的道口，高差较大的道口应加作引坡。
