　根据《中华人民共和国传染病防治法》和上级有关文件规定，为实现我院传染病报告率、及时率均达到100%，提高我院传染病报告管理工作质量，特制定如下制度：

　　1、各门诊科室均应设立门诊日志，病房（即住院部）应建立住院病人出入院登记本。所有门诊、住院部、检验科、放射科均应建立传染病登记本并备有传染病报告卡。

　　2、门诊日志项目至少要包括：姓名、性别、年龄、职业、住址、诊断病名、发病日期、就诊日期、初诊或复诊九项基本内容。门诊日志应由临床医生填写。病名项目应填写诊断的病名，不能填写症状。如属法定报告传染病，各个项目均应填写完整，且住址要求详细到村组或xx路xx门牌；如属非传染病，至少应记录姓名、性别、年龄、诊断病名和就诊日期等五项。

　　14、质量控制：疫情管理班子每旬对传染病疫情报告情况进行一次自查。
