　 舟山市危险化学品隐患排查治理（专项整治）工作方案

　为继续推进全市危险化学品隐患排查治理工作深入开展，进一步消除各类安全事故隐患，预防和减少事故的发生，建立健全安全生产隐患排查治理工作长效机制，实现事故隐患排查治理工作制度化、规范化、经常化。根据《浙江省“安全生产年”深化危险化学品隐患排查治理（专项整治）工作方案》和市安委办《关于“安全生产年”深化隐患排查治理工作的实施意见》的要求，现就我市危险化学品安全隐患排查治理活动，提出以下工作方案。

　一、工作目标

　　（一）通过深化隐患排查治理，推进《国务院安委办关于进一步加强危险化学品安全生产工作的指导意见》（安委办〔2008〕26号）各项安全措施的落实，逐步提升我市危险化学品领域本质安全水平。

　　（二）通过深化隐患排查治理，进一步落实企业安全生产主体责任和政府的安全监管责任，建立完善安全隐患排查治理各项监管制度和企业安全生产隐患排查治理的长效机制。

　（三）通过深化隐患排查治理，有效消除一批事故隐患，努力实现全市危险化学品一般以上事故“零”目标。

　二、活动对象

　今年危险化学品安全隐患排查治理主要对象是全市危险化学品（包括剧毒化学品、成品油）生产、储存、经营单位，烟花爆竹批发经营企业，以及构成重大危险源使用企业。

　三、主要任务

　围绕今年隐患排查和专项整治工作目标的落实，其主要任务：

　（一）按照省局即将印发的《关于规范全省危险化学品企业安全生产监督检查工作的意见》，各县（区）、有关管委会对本辖区企业按风险程度定性划分确定A、B、C、D四类企业监管等级，并公布企业分级名单，制订检查工作计划，全年实施监督检查工作。分级监管企业名单公布、检查工作计划4月中旬完成。

　（二）督促企业建立并完善隐患排查治理工作制度，严格制度化开展隐患排查治理工作。各县（区）、有关管委会要召开相关企业参加的开展隐患排查治理工作动员部署会议，要求企业层层落实安全生产责任制；修订和完善安全管理制度（包括安全检查及隐患整改制度）和安全操作规程；组织岗位操作人员安全培训教育。督促企业切实落实《浙江省化工建设项目试生产安全管理规定（试行）》、《浙江省化工有限空间作业安全管理规程（试行）》、《浙江省化工装置检修安全管理规定（试行）》等规定，进一步规范企业安全作业行为。完善并实施重大危险源“两牌一箱”、事故警示牌、安全隐患整改告示牌和化学品事故专项报告等四项制度。动员部署工作3月底前完成。

　（三）推行企业安全生产工作情况书面报告制度和隐患排查治理统计报送制度。各县（区）、有关管委会设计表格形式按照分类监管原则，对不同监管等级企业分别按季、半年、年度实施安全生产工作情况书面报告制度。报告主要内容：安全生产总体情况、安全隐患排查治理情况、主要设备设施运行及检测检验情况、人员培训教育及人员变动情况、建设项目实施情况等与安全生产管理相关情况。每季度首月10日前报送隐患排查治理统计报表。各县（区）、有关管委会根据企业安全生产管理情况进行针对性督促检查。

　（四）督促企业全面落实危险化学品建设项目安全“三同时”工作。严格新改扩建危险化学品建设项目设立、设计和竣工验收审查以及试生产（使用）备案等工作程序和内容要求，规范审批程序。建立化工建设项目动态监管跟踪机制，加强对化工建设项目安全生产“三同时”跟踪督查，严肃查处违法违规建设项目。

　（五）积极推进危险化学品从业单位安全标准化工作，提高安全标准化活动企业参与率，力争所有的危险化学品生产企业和50％以上危险化学品经营企业开展安全标准化活动。

　（六）严格安全生产许可证换证条件，积极做好开展危险化学品使用许可准备。在许可证换证过程中，严把换证条件，对不再具备应有安全条件的危险化学品生产企业坚决不予换证。积极开展使用危险化学品单位的调查摸底，掌握基本情况，了解行业整体安全生产状况，为实施危险化品使用行政许可做好准备。

　（七）打击非法生产经营活动和继续关闭不符合安全生产条件的化工生产企业。对未持有安全生产、经营许可证件从事危险化学品生产经营活动的企业，必须责令其停止生产经营活动，依据相关法律法规对其进行处罚，并提请当地政府实施关闭。

　（八）积极部署加快推进化工危险工艺（装置）自动化安全控制系统的安装工作。按照国务院安委办（安委办〔2008〕26号）和省局（浙安监管危化〔2008〕200号）要求，各县（区）、有关管委会要制定本地推行自动化安全控制系统的工作方案，落实每一家企业安装自动化安全控制系统的时间表。4月底前，各县（区）、有关管委会上报2010年前需安装自动化安全控制系统的企业名单和推进工作方案。

　（九）加快开展危险化学品登记工作。省局拟在3月份出台《浙江省危险化学品登记管理规定》和《浙江省开展危险化学品登记工作方案》，力争在4份启动登记工作。各县（区）、有关管委会要积极做好本辖区危险化学品登记单位的组织、协调和推进工作。

　（十）推进危险化学品道路运输安全监管联控机制建设。落实国务院安办《苏浙沪危险化学品道路运输安全监管联控机制建设推进会议纪要》和《苏浙沪危险化学品道路运输安全监管联控机制建设推进工作方案》要求，建立我省跨部门、跨行政区域危险化学品道路运输安全监管联控机制。省安委办将印发《浙江省实施苏浙沪危险化学品道路运输安全监管联控机制工作方案》，各市安全监管部门落实检查道口或主要公路附近危险化学品企业作为较大隐患车辆危险化学品应急处置卸载基地，各级安全监管部门按照属地原则督促危险化学品生产、储存、经营企业严格执行查验、充装、发货相关安全管理制度。8月底前完成。

　四、工作安排

　按照属地原则，各县（区）、有关管委会负责辖区内企业隐患排查治理监督检查工作，市安监部门负责监督指导工作。主要工作安排如下：

　　（一）危险化学品企业必须完成的主要工作

　1.逐级签订安全生产责任书；及时修订和完善安全管理制度（包括安全检查及隐患整改制度）和安全操作规程。

　2.组织岗位操作人员安全培训教育，举行全员反“三违”宣传活动，主要负责人至少给员工上一堂安全教育课。

　3.每季度开展一次以上全方位的隐患大排查；涉及硝化、氟化、氯化、重氮化、加氢等高危工艺的化工生产企业和剧毒化学品生产使用企业要自聘专家进行重点安全检查，查找在生产工艺、单元操作、重点反应装置、应急救援预案等方面存在的隐患和不足。

　4.加强对排查到隐患立即组织开展治理，对存在重大隐患要严格按照治理目标、经费、机构、时间和安全措施“五落实”要求进行整治。

　5.建立详细的安全检查记录，建立隐患排查治理台帐。

　6.符合挂设事故隐患告示牌条件的、构成重大危险源场所的，要按省局规定挂设事故隐患整改告示牌和重大危险源安全警示牌，并落实整改或重大危险源监控措施。企业3月底前完成动员部署工作，4月底前完成第一次全面排查工作，并向当地安监部门或辖区管委会报告隐患排查及相关工作落实情况。

　（二）安监部门须完成的主要工作

　按照本方案要求，因地制宜，有计划、有部署、有重点对开展危险化学品安全隐患排查治理的督促检查工作。须完成以下主要工作：

　　1.迅速部署，将隐患排查治理工作做到层层落实，督促企业尽早、认真开展隐患自查自纠。

　2.组织对企业开展的自查自纠进行督查，帮助、指导、监督企业及时落实工作要求；检查发现企业存在较多或重大隐患的，按D类或C类企业由省或当地政府挂牌督促限时整改；对未落实工作要求、未及时落实措施治理隐患、未制订整改计划、未排查出隐患等情况的企业要给予通报，违反法律法规的要依法予以处罚。根据督查情况，按照“百千万”企业安全服务专项行动要求，确定服务企业名单，明确安全服务重点。

　3.查处打击危险化学品的“三非”单位；对存在重大隐患的单位，及时督促要求做到“五落实”，并进行跟踪问效、监督问责，视情责令局部或全部停产整改。

　4.督促企业对危险作业场所、环节安全管理，按照省局四个技术规范性文件落实安全作业措施；督促企业认真执行“重大危险源安全警示牌”、“事故隐患整改告示牌”等四项制度。

　　5.根据《关于推行化工生产过程自动化安全控制系统的指导意见》计划，分类引导企业开展化工自动化安全控制系统升级改造，鼓励企业选用合适的DCS系统、报警联锁装置、紧急泄压装置、紧急停车系统等自动化安全控制设施。

　6.开展使用危险化学品的生产企业调查摸底，掌握企业底数、了解基本情况，为实施危险化学品使用许可作好准备。

　7.按照要求建立完善安全隐患排查（检查）相关工作制度、六类台帐和安全检查记录，结合安全标准化活动、安全生产许可证延期换证和日常检查情况，对企业进行分类建档、分级监管，并加强档案管理。

　8.汇总企业自查自纠情况，及时统计分析通报情况。6月底前，各县（区）、有关管委会向市局危化处报送企业隐患自查自纠查出隐患统计台帐及其他工作落实情况的阶段性总结。

　9.及时督促并指导被依法责令限期或停产整改的企业，在完成隐患整改后，组织技术人员和专家对隐患整改情况进行评估；安监部门根据评估结论和企业申请，及时对隐患进行销号。对逾期仍未完成整改或整改不符合安全要求的，一律依法予以继续停产整改；对整改无望的，及时提请当地政府予以关闭或督促企业实施停产搬迁。

　　10. 各县（区）、有关管委会要经常性地开展隐患排查治理情况监督检查。市安监局要在年中和年末组织开展2次抽查和督查，并通报抽查和督查情况。

　五、工作要求

　 一 积极动员，认真实施。各县（区）、有关管委会要进一步提高对深化隐患排查治理工作重要性的认识，消除松懈麻痹和厌战情绪，要按照市安委办深化隐患排查治理工作的总体部署，围绕专项工作方案十项主要任务，认真做好深化隐患排查治理工作的动员部署和监督检查工作，确保“安全生产年”危险化学品隐患排查治理工作取得实效。

　 二 突出重点，讲求实效。督促指导企业从“人、机、环境、管理”四个方面系统性开展一次全员“查身边隐患”“杜绝三违”隐患排查治理活动，真正将隐患排查治理工作作为员工的自觉行动。充分发挥专家和中介机构技术力量作用，有针对性指导督促企业深化隐患排查治理，狠抓危险化学品安全生产的薄弱环节和突出问题。要求具体工作措施坚决有力，确保不留死角、不留隐患；职责分工要明确，对排查出来的问题，切实落实整改，切实提高安全监管效果；对存在的共性问题和隐患，要积极研究制定专项安全管理措施，推进面上工作。

　 三 ?登记建档，分级监管。应按照“一企一档”的要求，建立辖区内危险化学品企业安全监督检查基础档案。建立和完善隐患排查治理的分级监管制度，落实企业和重大隐患的分级分层监管责任。完善企业监督管理台帐、日常检查记录台帐和隐患整改治理台帐，做到企业分布情况清楚、企业安全生产状况熟悉、事故隐患治理情况掌握。

　 四 ?立足治本，标本兼治。努力做到隐患排查治理工作多管齐下，以综合治理推进标本兼治，力求隐患排查治理与建立健全安全管理制度，促进依法生产经营与依法监督管理的有机结合、推进科技进步与提高安全技术水平相结合、产业结构调整与推动产业升级紧密结合、集中整治与建立长效管理机制互相结合。

　五 深入宣传，强化监督。充分利用多种渠道，开展隐患排查治理的宣传活动，宣传危险化学品相关法律、法规和标准规范；有效发挥广播、电视、报刊等新闻媒体的舆论监督作用，发动广大职工和人民群众积极参与并监督，形成有利的社会舆论氛围，促进隐患排查治理工作的深化开展。
